Patient Forms

Once you have made an appointment with Pure Health & Wellness Center, you will need to fill a variety of patient forms. To make it more convenient for you, you may download our required patient forms, print them, and fill them out prior to your appointment.

Please bring the following items to your first appointment. The more of your medical history that you have available, the more able the doctors will be to help you

• Bring your insurance card.
• Any previous lab work or imaging reports (x-rays, MRI, CT…)
• In the state of Arizona, you are not required to obtain a referral from you primary provider to see a chiropractor or naturopath
• If you were involved in an auto or work injury, please bring your claim number and any information about your case that you currently have available. We will let you know exactly what we need.
• Pure Health & Wellness accepts insurance for a wide range of services; please feel free to talk with the individual provider if you have any concerns about expenses. We will always work with you to in order to be able to provide you with the care you need.
